• Provide two pieces of I.D. Include one picture ID (Driver’s License, Social Security Card, Birth Certificate, Employee ID, Gov’t Veterans Card, Voter Registration Card, etc.).
• Provide Official High School Transcript/GED
• Click here to print a High School Transcript Request form
• Provide all academic transcripts
• Complete a placement test (2 hours)

To Apply to the Community Psychology or Urban Ministry Graduate Program.

You Need:

• Print the Graduate Application and the Graduate Attachment. Complete the forms and submit them to the Admission’s office.
